Activeandfit is $25/mo and recently got 24 Hr Fitness included again (went away 1/1/19 I believe it was). I've been a member for about 10 years, way better than any other option unless you have a cheaper local gym near you and happy with their hours, location, and equipment. Really can't beat the convenience of going to basically any gym anywhere in the US.
Where do you get the insurances rate or sign up that way?
Is 24 hour fitness still active with this program? Mines was recently cancelled
This offer is through American Specialty Health which runs activefitdirect, I'd assume it's the same network of gyms which is super extensive..... They do have "premium gyms" which cost more, i've avoided those, but think the orange theories, cycle bar etc...

Alternatively, you can get it through USAA Insurance or AAA (may vary by state).
Don't have the links handy, but there's a large thread on Reddit about USAA and Activeandfitdirect
Note that Active&Fit Direct and Active&Fit Now are two distinct offerings from the same company.
This thread has some useful info also:

There's no way to convert your Aetna signup into the AAA. You would have to cancel the first account and sign up with a new account through AAA. This also means you'd have to pay the signup fee again.
I did this last year, but it made sense because I had cancelled my membership while all the gyms were closed. Rather than re-upping, I created a whole new account. It took a few days to get it all sorted out because of email addresses and stuff, but I now will receive a $30 rebate every 12 months.
